| New providers please download and fill out the medical client profile and fax back to us, this allow us to load your facility into the system to link patient results when completed. Also find a blank requisition form available for download. Please use this form when ordering blood work for your patients. You may fax in both forms to 702-233-1793 or email them to laborders@aslvegas.com ** very important ** we emphasize that all lab orders now need to go to this email. Upon receipt of this information, you will be contacted with your secure portal login to view patient results electronically.
